Sorry you are in this situation.

You need to get back in the home. Make her leave until she agrees to no contact for life. You need to be firm on this and let her know you mean business. Otherwise you are enabling the affair. That is part of the stick of plan a. Waywards demonstrate an attitude of entitlement and you've been accommodating hers.

You also need to complete exposure on all sides. Did you specifically ask the people you exposed to to support you by persuading your wife to leave the affair? What has your pastor done to support you?

Have you confronted the other man? If no, why the heck not? Drive this low life away.

Have you identified her needs and have you made changes to meet them? You need show her that you are a good, attractive alternative to the other man while she is in the fog.
