Presently separated how do I find out where he lives and his phone records to prepare for divorce?

If you can't find your H to serve him there is usually the option of Service by Publication:

If you are in the US and he isn't then you should be able to file when you meet your state's residency requirements. Consult an attorney about the process. Many give a free consult or you can talk to a court's self help desk.
