In my "plan" I laid out for my H, I said I would write the OM a letter asking for NC. I know I have seen places on the forum to help with writing one, but I searched and I couldn't find anything. Any one know where I can look?

Dorry, thanks for the help. I am going to e-mail my H what I composed (or rather, what I borrowed from everyone else). As a FWW, if you can think of anything else that I should add, please let me know. Here is what I wrote...
(OM),
I wanted you to know that out of respect and love for my husband, I have come to realize that I must never see you or talk to you again. My relationship with you was a selfish and inconsiderate indulgence that my husband did not deserve. While I cannot completely repay Chris for the pain that I have caused him, I want to do my best to become the wife he has been missing and the wife that he so greatly deserves. I know that marital reconciliation with my husband is the right thing to do. I care a great deal about him and would not want to do anything to risk his happiness. I want things to work out with him so that we can have a family and realize all of our dreams together. I will not make any further contact with you and I do not want you to make any further contact with me. Please respect my desire to end our relationship.
