Try this...

Dear Company X,

My husband (Full Name), title, who is currently employed with your company is currently having an extramarital affair with OW (Full name), who is employed by (Company name), a supplier of ??? to your company.

I love my husband and would like to be able to remain married to him and recover our marriage. Clearly, this is impossible if he continues his affair with OW.

I respectfully request that you take appropriate action to ensure that OW (Full Name) no longer has contact with WH (Name) in his employment responsibilities.

For the OW's employer it could read this way.

Dear HR Departement,

An employee of yours, OW, has entered into an extramarital affair with WH, who works for a client of your company and is a married man with children.

Respectfully request that OW be removed from any account that requires her to provide services to (Company name).

Just a suggestion.

Yes,
Human Resources, his superviser, the President of the Company need to know that it is the wife who is asking them to follow whatever policies and procedures they have related to sexual relationships between employees and between employees and contract agents. They also need to know that as the betrayed wife in the situation, you are asking for their assistance to use whatever resources that are available within the corporation to assist you in saving your marriage.
