I sympathize with your situation. The best advice I can give you is to say "I want us to live a long and happy life together and I am concerned about your weight having an effect on your health. If your happy that's fine, but can we try working out together to get some exercise." I don't recommend gyms, diets or anything of that nature, fix smaller plates for each other, exercise together (ever if weight training), cut down on carbonated drinks and some weight will come off.
Take Care
Kie
