Does anyone know if there is a form I can print out. Our counselor has asked us to sign one and I was pretty sure I saw a form at one time. Thanks. Yes, the counselor is helping though there is lots of work to be done. L
There's no form, per se. This "policy" is described here, under the Concepts section.

Thanks, I have been reading more this afternoon, in concepts, and have printed out many of the articles that are in there. Maybe we will just read it and both sign it. These concepts should be used as a cover page on marriage licenses! L
