Although I've never ever cheated on my amazing wife, I've lied about fulfiling my own sexual needs with masturbation. We've had several major discussions in the past and I'd promised her to tell her the truth about it, but I was usually asked during a difficult/stressful time in our marriage (e.g. when her emotional needs weren't met), and would choose the easy way out by not disclosing the truth. I would convice myself that I was doing a private thing with myself. I'm embarrassed about occasionally fulfilling my sexual needs alone, and I know my wife looks at me differently (e.g. in disgust), when she knows about my private activities. I know in my heart that lying is a hundred times worse than dealing with the issue with her, but I'm always scared of her reaction when I tell her the truth.

What steps can I follow to completely change my ways so I never feel the need to cover the truth again?

Sincerely,

Well, isn't that sort of a private matter unless it interferes with the sexual relations of a marriage? I don't think you should lie, but I also think she shouldn't be asking about it. Why would she even ask such a question?
